The Request a Consult form is only for your contact details. Please don’t include symptoms, diagnoses, or other health details. We’ll talk about what brings you in privately, by phone.
This website and form are not for emergencies. If you’re in crisis, call or text 988 (Suicide & Crisis Lifeline) or call 911.
What we collect
- Consult requests. If you use the Request a Consult form, we collect your name, phone number, email address, whether it’s OK to text you, whether you’d like to be seen virtually or in person, and the best time to call.
- Basic technical data. Like most websites, our web server records basic technical information (such as IP address, browser type, and pages requested) for security and to keep the site running. We use it only to protect the site.
How we use it
We use your consult request only to contact you about scheduling. Requests are delivered by email to our practice and are not stored on the website. We never sell your information, and we don’t use it for advertising or marketing.
Cookies and tracking
This website does not use analytics, advertising, or tracking cookies, and it does not use tracking pixels. It only uses cookies that are needed for the site to work and stay secure.
Who we share it with
We share information only with the service providers that host this website and deliver its email, and only so they can provide those services. We may also disclose information when the law requires it.

You can ask us to delete a consult request you sent, or ask what information we have from this website, by calling (904) 674-9334 or writing to us at the address below.
